What documentation is required?
Pennsylvania law requires all medical marijuana patients to have a valid Pennsylvania driver’s license or state ID, along with a working email address, in order to apply.
CannaMD has separate requirements regarding proof of diagnosis; however, this documentation is not required when you register online with the Pennsylvania Department of Health.
How much is the state fee?
The state of Pennsylvania charges an annual patient card fee of $50. (Note: This is separate from your CannaMD physician fee.)
Patients who participate in Medicaid, PACE/ PACENET, CHIP, SNAP, and/or WIC may qualify for a waived card fee. When patients login to make their annual payment, the Registry will ask if they participate in any of the listed programs. Patients should select PACE if they participate in more than one. Once patients answer “yes,” their balance will go down to $0.
How do I register & create my account?
You’ll need to create your online account with the state before visiting CannaMD. Fortunately, online registration is easy!
Step 1: Have your Pennsylvania driver’s license or state ID ready. You’ll also need a working email address.
Step 2: Visit the Pennsylvania Department of Health Medical Marijuana Program Registry website.
Step 3: Under “New Users,” click the patient or caregiver link that best describes you (example: “Adult Patient Registration” or “Adult Patient Caregiver Registration”).
Step 4: Complete the registration form and follow all on-screen instructions.
Tips: Match the information exactly as it appears on your ID (exception: only use the first initial for your middle name, regardless of how it appears on your ID). Only enter digits where numbers are required (don’t include spaces or hyphens).
Step 5: When prompted, create a username and password.
Step 6: At this point, you should see a screen that displays account information. Make note of your Patient ID. You’ll need to share this number with your CannaMD doctor.
Step 7: You’re officially registered with the state! Check your email for a message from the Pennsylvania Department of Health concerning the next step (which is to obtain a patient certification from an approved practitioner, like those that work at CannaMD!).
How & when do I pay the state fee?
You’ll pay the required state fee after obtaining your physician certification from CannaMD. To pay, simply:

Note: Usernames and passwords are case-sensitive.
Step 3: Click the “Make Payment” tab and enter your payment information.
How do I know which caregiver option to select?
If registering as a caregiver, you’ll need to select the option that best describes your situation:
Adult Patient Caregiver Registration: You are registering for an adult patient who is already in the system. You will need to enter the patient’s name, date of birth, and Patient ID number when registering.
Caregiver Registering for an Existing Patient: You are registering for a patient who is already in the system but will not visit a dispensary. The patient will not register separately, nor will they receive an ID card in this instance. You will need to enter the patient’s name, date of birth and Patient ID number when registering.
Caregiver Registering for a New Patient: You are registering for a patient who is not yet in the system and will not visit a dispensary. The patient will not register separately, nor will they receive an ID card in this instance. You will need to enter the patient’s name, date of birth, address, and telephone number when registering.
DOH-Approved Caregiver Registration: When you register under this category, you are agreeing to allow the Department of Health to share your contact information with patients in your area who need a caregiver.
How do I register a minor?
Patients under the age of 18 require a caregiver to register on their behalf.
If you are the minor’s parent, legal guardian, or spouse, you can register as their caregiver by selecting the appropriate caregiver option on the registration page.
If you are not the minor’s parent, legal guardian, complete the online caregiver registration and then fill out an “Authorization to Designate a Third-Party Caregiver” form. This form can be downloaded from your caregiver profile.
Once completed, you can then upload the form into your caregiver profile.
How do I register a disabled patient?
If the patient is under the age of 18, they are required to have a caregiver apply on their behalf.
If a patient is 18 years of age or older and able to register themselves, they may do so.
If they cannot, a caregiver may register for them.

Where do I find my Caregiver ID number?

As a patient, how do I link myself with a caregiver already in the system?

As a patient, how do I link myself with a caregiver who wants to register?
Give the caregiver your name, date of birth, and patient ID number. The caregiver will enter this information when they register online.
As a caregiver, how do I link myself with a patient already in the system?
Step 1: Before registering, get the patient’s ID number and date of birth.
Step 2: Enter this information on the registration screen.
Note: If you have already registered and are not linked with the patient, the patient will need to sign into their account and click the “Manage Caregivers” tab to add you.
If you are not associated with a patient who has an active certification, you will not be able to purchase medical marijuana on their behalf at a dispensary.
What if I forgot my username or password?
Remember, usernames and passwords are case-sensitive.
If you’ve forgotten your username or password, visit the Registry login page and click “Forgot your username or password?” under “Returning Users.” This option will let you request an email with your username or request an email to reset your password.
Note: The link in the “reset password” email will only work once and expires 24 hours after requested.
